In an effort to “Keep Our Kids Safe” the Board of Christian Education has implemented a Medical Release Form for children and adults, and a Background Screening system for volunteers. These screenings are required for all Children and Youth Workers at no cost to the volunteer. Please note, Central Baptist will limit the background search to “criminal, court and police records” only. All information will be kept confidential, and comply with the federal government’s Fair Credit Reporting Act. Please complete all new forms and return them to the Church Office. Information will be kept on file for seven years, then shredded.
Parents need to complete a Medical Release Form for each child enrolled in the program. These forms cover the entire academic year (September 2009 to August 2010). New forms are needed each year. Information will be kept confidential.
